Puntland Needs A Health Architecture

The Health space in Puntland in all honesty has been a very under-performing sector, I doubt any Puntite will dispute that. Other then a few seasonal children immunization and great MCH coverage even rurally to reduce infant mortality, I cannot say much else has been done.

Therefore I propose the following Health Architecture.

1. Continue expanding MCH rurally untill infant mortality is within an acceptable range
2. Expand immunisation from this seasonal approach to a permanent approach and make it free/accessible at General Hospitals thru govt/donor co-pay
3. Expand General hospital access and coverage to every A and B district, thru Govt/Donor co-pay
4. Expand Specialist facilities for Common Problems that are treated overseas. Diaspora/Govt/Donor partnership
5. Invest in sewage/drainage/water treatment in urban areas to be world-class because lots of diseases are linked to hygeine
6. Invest in food-fruit Standards Plus Quality Enforcement as the food people eat is also another risk.

If we present the above health architecture and it's goals, we can break it down into programs and manageable goals and begin nation building our health sector, targetting district A-B as priorities.

Ideally in the end we want a health system that is affordable or subsidized or universally free so access isn't limited to people's wallets.

Health-care is a universal need therefore a universal right, it's not a choice. Healthcare is an important metric in economic development because life-spans means work-span. Unhealthy people are also unreliable human capital. PPL dying early is a social loss in 'knowledge retention' either thru life experience or work.

PL needs health revolution, I hope the presidential candidates On Jan 8, 2024 discuss this important area which is just as important if not more then education to a human population.

Once their delivered though they are vulnerable to preventable diseases thru immunisation which is only done 'seasonly' not 'regularly'.

Plus we have a very limited facilities to deal with senior citizens illness

It is well known in the health circles those two segments, the under 10 and the elderly are most vulnerable and need more govt investment to treat their illnesses.

The rest of the working age population require minimal intervention if those 'two' areas are heavily focused. Their is some in the working age bracket who have 'specific' problems(ears, eyes, joints, brain, etc) those are highly specialized areas and their usually small segment or 'few', our system will come back for them when we deal with the majority and especially the vulnerable segments among the majority.

I also suggest we set up a PL path-way immediately to Cuba who have cheap but world-class medical facilities and education. We must generate a quota of health professionals from our students either those with a passion or high scores or both after year 12 exams.

It's low cost on the student also in Cuba and it's a guaranteed path-way back to PL, because they won't want to live there unlike the west. This is only interim solution, ideally the govt needs to create facilities and medical education standards locally and bring professors here in the long-run to knowledge transfer and build up Puntland Medical capital.

If Farole sent a batch of 50-100 Health scholarships to Cuba in 2009, after 10-15 years of education/medical inhouse training, around year 2020-2023, lot's would be back and working in our hospitals.

Cracking down on FGM would probably save thousands of mothers and children a year. Somalia's infant mortality is much higher than it should be largely due to this.

I'm surprised this practice wasn't targeted during the Kacaan. One of the few things communists do well is cracking down on barbaric practices like that.

I want our health architecture to reduce infant mortality whether before birth or after(due to lack of immunisation) and increasing life-span by targetting investment into senior citizen infrastructure so they live longer. By following this approach we expand life-spans, cut infant mortality(birth related or child related). Then we can simply place general hospitals into our district A-B areas so the bulk working population who do get sick can access a GP. Then we can target specialized services later on based on 'prevalence' rates.
